The reality is that if two people come up and ask for a blessing, you don't really get in to the weeds about why they're asking: you just grant it.
That document is a joke and most bishops know it, though: that's why there's really not official diocesan policies dealing with these things.
The key here, though, is what the bishop is requiring of me. If a priest in good conscience cannot serve the Ordinary for whatever reason, it's probably time to move dioceses. This happens sometimes, because it's paramount that we are respectful and obey. At the same time, we can't have a conflict with conscience.
